Child/Adolescent Intensive (High) Care Coordinator

Location: Coffee County

Status: Full Time

Employees in this position are expected to be flexible, professional and a team player. Employees will coordinate services for high-risk youth involved with multiple agencies (school, DHR Juvenile Court, etc.) Conduct monthly meetings to address needs and goals. Participate in treatment team meetings with supervisor to identify progress/refer to higher level or lower level of care/transition to Low intensity care services. Coordinate needed services and link youth/family to services available in the community. Maintain billing documentation and active case plan. MUST HAVE Bachelor’s degree in a human services related field, ICC/CM trained preferred; current AL driver’s license, eligibility for coverage under center liability insurance and individually held vehicle liability insurance and clear background check.  

ALL candidates must be able to type and have basic computer skills, possess a valid AL driver’s license, eligibility for coverage under center liability insurance and individually held vehicle liability insurance with a clear background check.
