Case Management

Home-based support connecting seriously mentally ill adults and seriously emotionally disturbed children to the housing, healthcare, transportation, and community resources they need.

Service Overview

Whole-life support delivered in your community

Our Community Support Specialists are assigned to specific counties and visit consumers at home to conduct needs assessments and connect them to essential services. Case Management is primarily an outreach program — most services occur in the consumer's home and community.

  • Advocacy for services the consumer cannot access alone
  • Crisis intervention and coordination with mental health professionals
What We Offer

Services provided by our Community Support Specialists

Needs Assessment

A written comprehensive assessment of the consumer's assets, deficits, needs, and goals — completed within 30 days of enrollment.

Case Planning

A collaborative Plan of Care developed with the consumer and their support system, reviewed every six months.

Service Arrangement & Linkage

Coordinating contacts between the consumer and appropriate agencies or service providers through face-to-face, phone, or electronic communication.

Social Support

Helping consumers build or expand personal support networks through advocacy and connection with support groups.

Crisis Assistance

Actively supporting consumers through crisis situations and arranging professional crisis intervention as needed.

Monitoring & Follow-up

Ongoing evaluation of progress toward case plan goals and adjustment of services as needs change.

Who Qualifies

Case management for adults with serious mental illness

To qualify, you must meet SMI criteria, be at least 18 years old, have impaired role functioning, and show at least one marginally met life need. Services are not appropriate if symptoms are not expected to improve with case management or if you are already at maximum functioning. To request a referral, speak with your therapist.
