Service Overview

Mental health care where older adults live

The Geriatric Services Program delivers individualized mental health care at local nursing homes and assisted living facilities, eliminating transportation barriers for older adults. Services are designed to promote recovery and resiliency through a person-centered treatment process.

  • On-site services at partner facilities, Monday–Friday
  • Individualized mental health care and treatment planning
  • Focus on serious and persistent mental illness (SPMI)
  • Culturally sensitive access for consumers who are deaf or have limited English proficiency

Who Qualifies

Care for older adults with serious mental illness

The Geriatric Services Program primarily serves residents of nursing homes and assisted living facilities with serious and persistent mental illness age 62 and above. Individuals below age 62 will be considered based on their need for services. Admission criteria align with the Adult Outpatient program requirements.
