Supporting stability through housing connection
Stable housing is a critical part of overall health and well-being. South Central’s Housing Navigation services help individuals and families identify housing opportunities, overcome barriers to stable living, and connect with community resources that support long-term success.
Our Housing Specialist works alongside individuals who receive our services to provide guidance and support throughout the housing process. Whether someone is experiencing homelessness, housing instability, or difficulty navigating available resources, our team helps individuals move toward safer and more stable living environments.
Housing Navigation services are designed to support:
- Individuals experiencing housing instability or homelessness
- Individuals living with mental health or substance use challenges
- Families seeking affordable housing resources
- Individuals transitioning from crisis services or treatment programs
- People needing support accessing community housing resources

Application Assistance
Get help completing housing applications, gathering required documentation, and navigating eligibility requirements.
Community Coordination
Work with our Housing Specialist who can help coordinate with landlords, property managers, shelters, and partner agencies.
Supportive Referrals
Connect with additional services including behavioral health care, benefits assistance, transportation resources, and case management.
