In-Home Intervention

Two-person teams — a master's-level therapist and a bachelor's-level case manager — delivering intensive mental health care directly in your home, 24 hours a day.

Service Overview

Intensive care without leaving your home

The Adult In-Home Intervention program is a home-based treatment program provided by a two-person treatment team. Services are individualized, persistent, and designed to prevent psychiatric hospitalization or support transitions from inpatient care. Teams are available 24 hours per day and typically visit consumers weekly.

What We Offer

Full-spectrum support at home

Home-Based Therapy

Individual and family therapy delivered wherever you are most comfortable — no transportation needed.

Crisis Stabilization

Available 24 hours a day to respond to crises, defuse immediate situations, and stabilize the home environment.

Medication Monitoring

Ongoing monitoring and administration of medications to support treatment adherence.

Family Support

Psychoeducational services and family counseling to strengthen the entire support system.

Transition Support

Acts as a liaison with inpatient hospitals to ensure a smooth, supported transition back to community living.

Basic Living Skills

Practical skills training to promote independent community living and daily functioning.

Who Qualifies

For adults who need more than outpatient care

You may qualify if you have a serious mental illness (SMI), are 18 or older, and have a clearly documented need such as increasing symptoms, transition from a more intensive level of care, an immediate crisis, or difficulty engaging in outpatient services.
